Online Form Instructions

Before using either one of these forms you must first contact our Bistro to make sure we have the date and time available.  Once you have done that you can complete the appropriate form. 

Catering Form

This form is used for most catering orders.  It allows you to choose 10 different types of box lunches.  If you have more choices you can complete the personalized form which has 15 choices or submit multiple forms.

For each line you enter either a sandwich or salad order.

You can choose:

type of boxed lunch

(Sandwich or Salad)

bread or dressing

Quantity

Sides (up to 3 choices)

Then a comment regarding that specific choice

(Ex: No onion)

Once all the boxed lunches are completed you can also order beverages and put an additional comment.

Personalized Order Form

This is something that we offer that most caters do not.  We will personalize an order up to 15 people.

What does that mean?  Well many times smaller groups know exactly what they want for lunch.  You can have each of your guests complete our form or tell you what they want and then you can submit the order.  In the space provided you can put in their name and we will make their lunch specifically for them and then label their lunch with their name. 

For each line you enter either a sandwich or salad order.

You can choose:

Quantity

Sides (up to 3 choices)

Comment here is where you put their name and any special instructions.

(Ex: John    No onion)

Restaurant Hours:  Monday - Friday  10:00 am to 3:00 pm

 Salad Bar Hours:   11:00 am to 2:00 pm
